This is a great analysis but it is kind of missing one angle that is vital to mainstream charity - donor acceptability. A program might not outperform cash, but if it can raise cash from donors that wouldn't donate to a more effective program, it can still be worthwhile for someone to do.

Another reason a charity can outperform giving people cash is that a charity can be an institution, like a hospital or health clinic or legal office. If you give someone money they might want to spend it on healthcare but there might not *be* a health clinic in their area so they may be unable to convert it to healthcare.
